The aim of the HOPE program is to support people for a period of 12 weeks, providing assertive and coordinated care post-discharge, via both outreach and clinic-based therapeutic interventions to reduce the risk of suicide re-attempts and deaths by suicide.
The HOPE team is a multidisciplinary team based at the RMH Parkville. Each client is allocated a Psychosocial Support Worker and Mental Health Clinician to deliver practical support and evidence-based therapeutic interventions to address the unique, economic, historical and environmental factors that have contributed to, or continue to contribute to, a person’s suicidality. Over a 12 week period, they deliver individualized therapeutic care, psychosocial support, and safety planning to help reduce the risk of suicide re-attempts.
